If monthly Basic subscription lapses for longer than 90 continuous days, Gold subscription for longer than 60 continuous days OR missing two monthly installments in a 3 month cycle, or three monthly installments in a 6 month cycle.

Your provider will electronically submit your prescriptions to pharmacy of choice.
A generic lab order form or commercial lab order will be provided at the end of the visit. It is the patient’s responsibility to make sure prior to the visit and the results sent to VE to ensure they are available at the time of the visit.
